Screening and diagnosis

Your child's doctor will likely diagnose tonsillitis on the basis of a physical exam and a throat culture or other rapid diagnostic test. During a physical exam, your pediatrician will use a tongue depressor to get a clear look at your child's tonsils. He or she will check for redness and pus on the tonsils and on the back of your child's throat. Although these signs indicate an infection, there's no way to tell by looking whether it's viral or bacterial.

In most cases, your doctor will also take a throat swab to check for the presence of bacteria. In this simple test, a sterile swab is rubbed over the back of your child's throat to get a sample of the secretions. It's not a painful procedure, but it may cause momentary gagging.

This sample may then be checked with the aid of a rapid strep test for the presence of streptococcal bacteria. Streptococcal bacteria can cause tonsillitis and strep throat, and the signs and symptoms of these illnesses are similar. A severe sore throat can also be caused by diphtheria, though this occurs rarely because most children are vaccinated against this bacterium.

Within minutes, the rapid test may indicate a positive result for streptococcal bacteria, in which case antibiotics will be needed to treat the infection. If the test is negative, your doctor may send the sample on to a laboratory for further testing. It's possible that the rapid test isn't sensitive enough to pick up bacteria that could be behind a sore throat and inflamed tonsils. There's also a test that checks for the presence of streptococcal DNA. This test takes several hours.

Based on the results of diagnostic tests, your doctor can determine whether a virus or bacterium is to blame for inflamed tonsils. This is crucial information because viral infections don't respond to antibiotic therapy.
