Treatment

Treatment for PAD has two major goals. The first is to manage symptoms, such as leg pain, so that you can resume physical activities. The second is to stop the progression of PAD, to reduce your risk of heart attack and stroke.

Many people can accomplish these goals with lifestyle changes. But lifestyle changes may not be enough. Your doctor may prescribe drugs to prevent blood clots and to lower blood pressure and cholesterol levels. He or she may also recommend anti-platelet medications — such as aspirin — which help prevent blood clotting. In addition, other medications such as cilostazol (Pletal) or pentoxifylline (Pentoxil, Trental) have been shown to improve claudication.

In some cases, other interventions may be considered for intermittent claudication. These may include:

  • Angioplasty. In this procedure, your doctor inserts a balloon-tipped, long, thin tube (catheter) into the blocked or narrowed part of your artery. The balloon is inflated, compressing the plaques against your artery walls. A mesh tube (stent) may be left in the artery to help keep the artery open.

  • Vascular surgery. Your doctor may create a graft bypass using a vessel from another part of your body or a tube made of synthetic fabric. This allows blood to flow around the blocked or narrowed artery.

  • Thrombolytic therapy. If you have an artery that's blocked by a blood clot, your doctor may insert a clot-dissolving drug into your artery at the point of the clot to break it up.

These interventions usually require angiography — a procedure in which a small catheter is inserted into your arteries and filled with dye, allowing your doctor to see narrow spots and blockages with the help of X-rays. All three interventions are typically reserved for severe cases of intermittent claudication that inhibit a person's ability to work or complete everyday activities.
